ChantGrammarElement SetAttribute fails

Symptoms

After creating ChantGrammarElements, SetAttribute fails to set attribute value.

Cause

XML element was not instantiated even though the ChantGrammarElement object was instantiated. XML element instantiation occurs when appending or inserting as a child element.
